My Care Wellness Visit
Your journey toward wellness begins with your My Care Wellness Visit. This appointment is the first step of your participation in the Community Cancer Center’s wellness program.
Your journey to wellness does not happen alone. Your family and loved ones who support you (whom we refer to as your family caregivers) are an integral part of your wellness journey. We encourage you to bring members of your support team with you.
The My Care Wellness Program is available to any cancer patient, regardless of where you received your treatment, if you are newly diagnosed or several years out and is available at no cost to you. The program is funded through private donations and represents the Community Cancer Center’s commitment to providing services that improve and enhance the quality of life of patients and their family caregivers. This includes addressing nutrition, fitness and emotional matters.
Preparing for your My Care Wellness Visit
You’ll begin by meeting with one of Helen G. Nassif Community Cancer Center’s Care Coordinators. Your Care Coordinator will help bridge and connect you and your family caregivers with resources within the Cancer Center, at UnityPoint Health – St. Luke’s Hospital, Physician’s Clinic of Iowa and in the community as a whole. Our goal is to educate and provide resources to you and your support team that will support you on your journey to wellness.
At your My Care Wellness Visit the Care Coordinator will:
- Identify your unique goals and concerns related to wellness and life after treatment
- Identify any concerns and support needs for your family caregivers
- Educate you on support services, programs and resources available to you and your family caregivers
- Coordinate referrals for support services you choose as part of your personalized plan
- Establish a follow-up plan to continually address your changing wellness needs

A legacy of helping OTHERS
